Healthcare Exemption Form Information

When it comes time for tax season, we understand it's essential to obtain all the facts and answers to your tax questions. Here are some quick resources and important FAQs regarding OneShare Health and Healthcare Exemption Form Information.

As with all HCSMs under 26 USC § 5000A(d)(2)(B)(ii), OneShare Health’s Members are exempt from the ACA individual mandate requirement of the U.S. Patient Protection and Affordable Care Act ("ACA"). However, as of January 1, 2019, the ACA individual mandate is no longer in effect.

However, some states have implemented a state individual mandate. Members who reside in California, Massachusetts, New Jersey, Rhode Island, or Vermont are required to provide proof of Health Coverage Exemption (formerly utilizing the federal 8965 Health Coverage Exemption Tax Form) if they are not enrolled in insurance. Members can find information needed to complete Health Coverage Exemption Tax Form(s) via their OneShareBox dashboard under the section titled “Tax Information.” If a Member is requesting their 2020 Healthcare Coverage Exemption Form information, please direct them to log in below.

How do I log into my OneShareBox?

If you have not logged into your OneShareBox before, you will need to create an account– username and password– and you will need your OneShare Health Membership number to do so.

 

Where can I find my Health Coverage Exemption Information?

Upon logging into your OneShareBox, you will be presented with your OneShareBox Dashboard. On your dashboard, you will click on the Health Coverage Exemption Information button within the “Tax Information” widget. If you are accessing this information for the 2020 tax year, make sure that 2020 is selected from the drop-down menu.

 

What Health Coverage Exemption Information am I looking for?

Information includes names of family members on your Program, total amount of Contributions for the tax year, number of months in 2020 that Members were covered and the Exemption Code for a Health Care Sharing Ministry: “D.” In addition, the Member can download this information in a PDF document.
